How to Register with the Practice

We are currently accepting new patients who live within the G64 postcode area.

To register with the practice, you will need to complete two registration forms:

Registration Form – Part 1

Registration Form – Part 2

Both forms can be downloaded from our website, completed electronically, and emailed to the practice.

Important information before you register

Please ensure you have at least a 30‑day supply of medication issued by your current GP practice before registering with us. This will ensure you have sufficient medication while your registration is being processed by the practice and NHS Greater Glasgow & Clyde.

If more than one family member is registering, separate registration forms must be completed for each patient.

Saving your forms

When saving your completed forms, please select File > Save As and rename the file before emailing it to us.

You may need to download Adobe Acrobat Reader to complete and save the forms correctly.
